Registration Information

Registration for the 2009-10 school year is currently underway.  ACME continues to accept new students throughout the year only in the following classes:
          Pre-K Music
          Elementary Music (K-2 & 3-5)
          General Music

To register for any of the classes offered by ACME, you must complete and return a registration form and a Permission to Publish form.  If you need financial assistance in the form of a waiver, you will also need to complete and return the Tuition/Fee Waiver Request form.

Following are links to the ACME registration forms, as well as other forms that are used throughout the school year.  All forms will have to be loaded onto your computer to be completed.  Upon completion, you may either print and mail the form to us or email the form to us as an attachment, with the exception of the Tuition/Fee Waiver Request.  This form must be mailed or hand-delivered to Director Shirley Faulkner or a current board member.  (A list of members can be found under the "About ACME" link.)

Registration Form - Please complete a separate form for each student.

Permission to Publish - Please complete one form per family.  Be sure to include an adult family members who may participate in any classes.

Travel Permission Form - This form is required only when you are notified of the need to travel out-of-town or as a class to a location other than the regular meeting place.  Do NOT complete this form at registration.

T-shirt Order Form - ACME t-shirts may be ordered by anyone, be it student, student family member or just someone showing support for our organization.  They are $10 each.  All students in the performing groups are required to purchase an ACME t-shirt.  They are worn for all casual performances.  T-shirt orders are taken at the beginning of the school year and in January following Christmas break.  (Larger orders help keep the price reasonable.)
